Student Living Apart from Guardian
By signing the form, you agree to act as the responsible party for the child in school. You will be taking over the duties and obligations the student's parents would normally have for school purposes only. The form gives the school a responsible party (you) to contact with any questions about the child, or to send information home about the child. The school will have the right to ask you questions to make sure you meet the conditions set out in the form. Please note that the affidavit will need to be filled out and notarized at the beginning of each school year.
